Rotator Cuff Pain: Why Your Shoulder Aches Overhead

The rotator cuff is a group of four small muscles that keep the ball of the shoulder centred while you move. When one of them is overloaded or irritated, reaching overhead, putting on a shirt or lying on that side can become a nagging ache. For many people this is a manageable soft-tissue problem rather than anything torn, especially when it builds up gradually.

What the rotator cuff does

These muscles do not produce big movements; their job is control and stability so the larger shoulder muscles can work. Poor posture, repetitive overhead tasks or a sudden increase in gym volume can overload them, leading to pain at the side of the shoulder that is often worse at night. Because the area is hard to rest completely, it can grumble on for weeks if the load is not adjusted.

What usually helps

Easing off the aggravating movement for a while, then gradually loading the shoulder with gentle strengthening, is the backbone of recovery. Keeping the upper back mobile and the neck relaxed takes pressure off the area. Sleeping with a pillow to support the arm can reduce the night ache while things settle.

Where massage fits

Massage can ease the tight muscles around the shoulder blade and neck that often tag along with rotator cuff irritation, which can make movement more comfortable alongside a strengthening plan. It is not a fix for a true tear. If you have significant weakness, cannot lift the arm, or had a fall or dislocation, see a doctor or physiotherapist first.

Key takeaways

  • The rotator cuff controls and stabilises the shoulder
  • Most everyday pain is overload, not a tear
  • Adjusting load plus gentle strengthening is the core fix
  • See a professional for marked weakness or after an injury

Frequently asked questions

Is rotator cuff pain the same as a tear?

No. Most everyday rotator cuff pain is irritation or overload of the tendons, not a tear. True tears usually involve marked weakness or follow an injury, and need medical assessment.

Why is it worse at night?

Lying on the shoulder compresses the irritated tendons, and there is less distraction from the ache. A supportive pillow under the arm often helps until it settles.
